Become A Patient
Begin the process to becoming a patient of the Advanced Bariatric Center…your pathway to health. The following steps are necessary:

Attend an INFORMATIONAL SEMINAR. If you are interested in exploring surgical options, our seminars are held in the office every Tuesday at 2:00 pm. Guests are welcome to attend. The seminar lasts approximately 60 to 90 minutes. It is mandatory to attend a seminar prior to your surgery. Information on seminars held in Pismo Beach and other locations are available by contacting the office during normal business hours (see “Contact Us”).

Register with our office to begin the new patient process either by email (under "Contact Us") where you will be contacted by one of our office staff to arrange a preliminary appointment or by telephone ( toll free 1-888-554-THIN).

Download the informational booklet and patient questionnaire (found in the Downloads section under the Medical or Surgical Weight Loss pages). Read over the booklet and fill out the questionnaire before you come to the office for your initial visit.
