The following materials are available for download to patients considering surgery; either the Laparoscopic Roux-en-Y Gastric Bypass or the Laparoscopic Adjustable Gastric Banding. The Surgical Information Booklet describes all aspects of our surgical program, the surgical options, benefits, outcomes, risks, as well as how the process flows together. The Preoperative Patient Questionaire needs to be filled out prior to your first visit. Please take your time filling out the questionaire as this important information assists us in determining whether you are a suitable candidate for surgery.
